Hospice at Home service is available for patients aged 18 years or over: patient has specialist palliative care needs (e.g. management of symptoms, psychological support); patient wishes to remain at home as far as their illness and symptoms allow; carer requires additional support to better enable them to continue to care for the patient.

About St Michael's Hospice (North Hampshire)

We are an adult Hospice, a charity caring and supporting patients and their families needing end-of-life and palliative care. We care for people across North Hampshire both on our In-Patient Unit and in their own homes.
